Does ON428 line 6097 Ontario tax reduction for dependent with mental or physical impairment. apply to a spouse also?

Yes, ON428 line 6097 can apply to a spouse.  

From the guide:

"You can include a spouse or common-law partner with a mental or physical impairment if you are claiming a disability amount transferred from your spouse or common-law partner on line 3 of your Schedule ON(S2), Provincial Amounts Transferred From Your Spouse or Common-Law Partner." 

If the CRA has approved a T2201 application for the Disability Tax Credit for your spouse, make sure to indicate this in TurboTax by going to Deductions > Disability Amount.
