My 16 year old child started working part-time in 2014. Do I claim her income on my return or does she have to file her own?

Don't report her income. Just your own. That goes for everyone.

She should file her own tax return, but you can still claim her as a child dependant

If you don't report income on your own return then do you put 0 income for child when it asks you under dependents income was zero?
