Does a Blue Cross 'Claims Total Report", itemizing claims for the year & amounts paid by you, qualify as an "official tax receipt"?

Yes! Often times when you submit a claim to a Private Health Services Plan (PHSP or "extended"), you send the original receipts and the copies are not returned to you. CRA accepts this as an official receipt, but you must be very careful to not claim anything for which you have received a reimbursement for.
