Out of town surgery . what can i deduct ? wife also had to stay in hotel for 5 days waiting to drive me home

It would depend on the nature of the surgery. In general terms treatment at a licensed private hospital can qualify for the medical expense credit. 

Travel for medical purposes can also be claimed if the treatment was not available in the area of your home and you were required to travel more than 40 kilometers (one way) to claim transportation costs for the patient. If the travel was greater than 80 kilometers (one way) you can also claim accommodation and meals. If there was a doctor's prescription for someone to accompany the patient then travel-related expenses for the attendant could also be claimed.
