When you report the information for the child care provider:
You need the name of the company or the business
The SIN number of the daycare provider in the case of the Home Day Care (if the business is not registered) or the Nanny services
The amount paid
You will report the weeks if it was a camp or a boarding school.
Given this information> make sure the number you are trying to report is in "the name of the person or the company" Box, not the "SIN number" Box
