How do we report a health care premium when we receive a portion of the expense from a HSA?

We are recently retired and have a private health care plan with and annual premium. My wife also has a Health Spending Account (HSA) that provides some funds towards the premium. There is no T slip for the HSA. How do we show the cost of the premium and the money received from the HSA on our taxes.

How do we report a health care premium when we receive a portion of the expense from a HSA?

You can claim the net cost of the premium as a medical expense.
Net private health plan cost = Annual plan premium cost -HSA contribution

The usual rules apply to the sum total of your medical expense credit

Medical claim = (net plan cost (see above) + Unreimbursed eligible medical expenses.) LESS 3 % of your line 236 net income or $2208, whichever is lower.
