If you are in Form mode, use the Personal Information
Worksheet (search for INFOWS), and the Personal Information Dialog (search for
info, select PINFO) to enter this information.

Thank you for taking the time to respond. Unfortunately that did not work either. I had to go to basic mode, but even then am not able to answer all yes/no questions or input all information.
