Do I add the death benefit of my mother to my own T4A(P) in box 18 which was auto filled from CRA or do I enter a separate T4A(P)?

Adding it to my T4A(P) makes no difference in taxes paid. The additional T4A(P) significantly affects taxes due.

Sorry for your loss. You need to enter this on a separate T4A(P) slip as CRA has issued two separate slips for this amount. The end result should be exactly the same either way you enter them as these are both taxable incomes.
