You answer Yes to the question you want to claim the disability tax credit, then in the other boxes, it needs to know the following: 1st box: the amount of childcare and/or disability supports deduction claimed by you or anybody else

2nd box: total of his/her Line 303-315(if he/she files a tax return)

3rd box: Line 257 from their tax return(son/daughter)

4th box: provincial amounts L5812-5840(again if he/she files a tax return)

If none applies then leave all blank
