If you are a CRA My Account holder, your T4A(OAS) slip can be viewed in your CRA My Account. From the Tax returns tab, select the option for Tax Slips. Then select the tax year and slip type.
If you are not a CRA My Account holder, contact Service Canada to obtain a duplicate copy of your 2015 T4A(OAS)
