What do i input in the medical expenses line?

it asks for payment made to. Do i input the pharmacy name?
and also description.. do i input 'prescription'?

Credits and deductions

Provide as much detail as you require to identify the type of expense you are claiming. For example the name of a doctor, clinic or pharmacy will be sufficient. Your receipts themselves will contain all the information that CRA requires, should they need that information they will contact you by mail at a later date.
