Do I need to fill out a self-employment statement, if I have income in box 82 on my T4?

If the income is also reported in Box 14 you do not need to use the Statement of Business Activities form. If Box 14 is blank and there are no amounts in Box 16 or Box 18 then you will need to report the income on the T2125.
