Do I need to upgrade from Basic package to a Standard package because I have medical expenses? Is there an additional ost of $20 as the program indicated?

No. You don't need to upgrade to enter medical expenses. Click on the "Continue" button to continue with adding your medical expenses using Basic.

You can use the Find/Search (magnifying glass) icon to go to your medical profile and include your medical expenses.
