I'm a superintendent of a building and received T4 showing income. I don't pay rent, instead I get my rent in lieu of managing the building..

It shows I received $12000 in income but I didn't, I just didn't pay rent. So how do I file this supposed income/rent?

You enter the T4 as income as usual. It is considered taxable income as usual.

Yes. Your landlord should issue a receipt. (If in Ontario, Manitoba or Quebec.)
