No. You don't need to upgrade to enter medical expenses. Click on the "Continue" button to continue with adding your medical expenses using Basic.

Fortunately, you do not need to upgrade your TurboTax software. All our TurboTax versions support medical expenses. 

You can use the Find/Search (magnifying glass) icon to go to your medical profile and include your medical expenses.
