Received disability from my work insurance during an operation. What tax form should I get from them?

It depends.

If you received benefits from a medical plan, you'll usually receive a T4A.

With some types of medical payments, if you've been paying the premiums, you won't receive any slip as these benefits may be exempt from income. This is more common with long-term benefits.
