Can some one please tell what currency should be used in the page "Foreign Slip"?

I am assuming it should be in Canadian dollars using the official exchange rate, but I am not sure of that

Yes, you should enter the amount in Canadian dollars. You can get the exchange rate from this Bank of Canada webpage: Exchange Rates.
