for Canada caregiver care tax credit for my father physically weak 74 years old . I ask doctor to make Doctor Note , he was referring to T2201 form. im just looking for doctor note template but do I have to fill the  disability form T2201 ?

I have not seen a template but it does not require the T2201. 

A note stating that dad is 'infirm" is all that is needed. That dad requires your help.
