I would like to clarify about do I need to input my (dividend from Hong Kong) in the part of T-slip Foreign income summary and also T-1135 with same information? Thanks

The T-1135 is a verification statement form. Your dividend should be report in Canadian dollars as per the Income Tax Act (ITA) requirements and should go in your T4A-Statement of Pension, Retirement, Annuity, and Other Income.
