Looks like you installed TurboTax on your computer. Schedule 4 can be printed by going to View > Forms Method, View > Find Forms, type S4. Then print the form (File > Print > Print Form).

Schedule 4 has three sections. Section I is dividend income, Section II is investment income, Section III is for investment expense. Depending on what is on your Schedule 4, supporting info can vary. Attach any T slip (T3/T5/T4PS/T5008/T5013/T1206) and receipt (carrying charges, investment income not reported on a T slip) associated with your S4 and S4-Supplement to send to the CRA.

@dmtil123 Are you using TurboTax Online or TurboTax Download?
In TurboTax Online, you won’t see the “View” option. Instead, you can login and scroll to the bottom of the Welcome screen. There you’ll see links to the PDF archives of any returns that you’ve already filed. These archives contain all the forms that make up your return, including Schedule 4.
why is there no statement of investment income filed in my tax return?
@dmtil123 I apologize. In my previous answer I got confused between the schedules. Schedule 4 has not been part of the T1 tax return since 2017. You will see a list of the investments entered on your return in the "Other Income" section of your Tax Archive PDF, or you can go into your tax return itself and look at the slips and the Investment section to see what was entered.
