form TP-766-2

I have received the form TP-766-2. what applies to me is adjustment to my pension in 2018, paid in 2019. On the form it also states to check box 404. Once you do it brings you to another form, and this is where i am at a lost as to where would I enter the amount and ensure box 404 is indeed checked.

Thank-you. However, the form in question mentions to check box 404, which Turbo Tax does not let you do.

Will checking box 404 become possible in a future release?
