In order to qualify for the Disability Tax Credit (DTC), you must have an Authorized Practitioner complete and sign a T2201 form. This form must then be approved by CRA. Diabetics can qualify for the credit if they meet the requirements mentioned in the following CRA publication.

Please read the section on Life-Sustaining Therapy:  https://www.canada.ca/en/revenue-agency/services/tax/individuals/segments/tax-credits-deductions-persons-disabilities/information-medical-practitioners/eligibility-criteria-disability-tax-credit.html

The Federal Public Transit tax credit was eliminated in 2017, currently only Ontario offers a credit for seniors. https://www.ontario.ca/page/ontario-seniors-public-transit-tax-credit
