T5008 Form has no box numbers on it, I received it as a statement, how do I know what to put in each box on my tax forms?

My form came from the CRA as a summary. It has all of my transaction dates, quanities, prices and credit amounts but does not state what I should put in for my taxes. I also received a T5 with it. Do I only use the T5 statement?

    If you have received a T5008 Summary (no box numbers), you can enter this by going to Income > Investments > Capital Gains & Capital Gains Deduction Profile. Then check off the box "Sold stocks, bonds, real estate ,etc ", and continue to the Capital Gains page to make your entries. 

    The T5 you were issued may relate to the same investments but needs to be entered as a separate slip. You may see on that slip "capital gains dividends" that do relate to this investment, yet they still need to be entered separately.
