Where do I find the new Schedule 13 form for 2018 which is Employment insurance premiums on self- employment and other eligible earnings. CRA website its form 5000-S13

In the "Self-Employment" section of your return after you have completed the "Profile" section. 

• Under the Income tab, drop down to Self-employment and start from there, 

•  Once you get to the Expense section you will see a screen that states "How much did you pay in Salaries, Wages, and Benefits?" this includes your EI Premiums.
