Can you please provide me with an example of a completed Northern Living Allowance tax form?

I am having difficulty completing the T2222 - Northern Living Allowance form. Specifically, the section for travel expenses.  Can you please provide me with a completed example showing what to place in the boxes to claim a vacation expense.
