I am working for a placement agency who did not give me a T4 and does not make any conteributions to CPP or EI where do I report income

Do I report this as Self Employment earnings and then pay CPP premiums including the employer portion??

You're likely a self-employed contractor if you didn't receive a T4 from your employer. You'll file a T4A to report your income and expenses there.
