Should i click YEs or NO for the 1st question whether i made contributions to RRSP plan ( my husband is the contributor but i am the annuitant...)

You would say "yes" for your husband as he bought the RRSP.  He will enter the rrsp under his name and enter it as a spousal contribution.   He will get the deduction on his tax return, as the contributor and you own the plan.
