How to claim retail commission?and which address on T2125

I'm working for a retail store and got commission from one of the store's suppliers (not from my employer).on my T4A I have it as Service fees,Box 48.should i claim it as Biz fees or Professional fees on my T4A and what would be the address on T2125 (Mine/employer or Supplier)?

You would write your retailer store address since that is where you work from. You would claim as professional fees. From what your saying you are not a business owner but a worker at a retail store earning commission.

I should add on my T4A is supplier's address not my employer
