If I make income from a T5 form, do I also include that same income in the general 'how much have you made from professional income?' section? or is that now double income?

What kind of income do you have on the T5? If you have received dividends from an incorporated business that you own, then you only need to enter the T5. You don't need to fill out the self-employment forms.
