Why do i get a smaller refund than my spouse who makes more...and i have credits such as medical, child fitness

The refund or balance due is arrived at by calculating the tax liability, and comparing that to the refundable credits.  The difference is either a balance due or a refund.  The difference in income is not going to be a factor as much as how much monies were withheld for taxes compared to the actual tax liability.
