Treatment of form 1099-DIV

I am wondering whether I need to include form 1099-DIV on my canadian tax return.  The income relates to plan distributions from an employee share savings plan.  I only see Box 1a and 1b on the form.  The payments were made in Canadian but are being converted to USD on the form. Does this need to be reported as foreign income?
