Cannot find form AB428 on my 2021 tax return

I viewed my detailed tax summary and clicked on the arrow below my name to see that summary as well. I cannot find the AB428 form as I need it to add totals together to give my parents as I am a dependent. Where could I go about finding this form with the lines shown on it?
