CRA is asking for a completed T778-what do I send off?

Hi,

The CRA is asking me for a completed T778-Child Care Expenses Deduction form. I printed off the completed form from the PDF. Is this acceptable for the CRA? If not, how can I get a proper completed form for the CRA?
