what box do i enter an amount? and do i enter monthly or total amount since receiving GIS

You will get a T4A(OAS) that will show how much you got for both Old Age Security & Guaranteed Income Supplement. If you have a Canada Revenue Agency’s (CRA) My Account service login, it should be available there.
