My income is zero for the current tax year. Should I still report medical expenses to carry forward to next year?

No, you don't need to enter your medical expenses if you don't have any income.
Medical expenses can be claimed in a 12-month period with the final month ending in the tax year you are filing. So, you might still be able to claim your unused expenses next year.
