I receive last week a message from CRA
-a copy of all information slips used to calculate income tax deducted (is this all the T4)??
-a signed T1032 form (I am single)??

Yes, the information slips used to calculate income tax deducted would be your t-slips.
If you don't have a spouse, you can send a note explaining this instead of the T1032.
