Tax whithheld and dividends?

I worked in Canada for a USA company where I have shares. I received a 2017 tax reporting statement with Total ordinary dividends and qualified dividends also a federal income tax withheld.  How or where do I report this amounts in my income tax?

Tax whithheld and dividends?

Dividend income from USA are reported and taxed as ordinary income in Canada. Therefore you should report it as foreign income in our T5 and deduct the tax paid as well. Btw, is the slip you received a USA 1042s? If it is use the T5 to report the income in Canadian dollars.
