How do I fill out the form with two jobs?

I worked in a restaurant since October. I just started a new full-time job. I never received a tax return form from the restaurant job to fill out, and my new job gave me one. I plan on keeping both jobs but working full time at one and only working at the restaurant job one night a week.

If you are considered an employee, the restaurant should have issued you a T4 slip. You may want to contact them regarding this issue.

Otherwise, you can still report the income under "Other Income" that is not reported on a t-slip.
