Doing variable amounts to plan, What box and t slip does an RRSP lump sum withdraw get entered into?

Going in circles trying to find info on lump sum withdraws.   Calculating tax and GIS losses is looking like RRSP was a mistake. Lump sum may be best option.  Need  the t slip box info to continue to estimate the hit.

You should receive a T4RSP - Any amount withdrawn that is more than the withdrawal limit must be reported in box 22. 

RRSP withdrawal amounts, including any tax paid on the withdrawal, are included in taxable income on Line 12900 of the tax return (RRSP income).
