I do not know what to enter on Box 20 of the T5008 form as this was not provided to me by the Broker and they do not have this information.

Is the amount a dollar value of the amount of the shares or the amount of the share price (average) multiplied by the number of shares that I had at time I gave to the broker?

Box 20 is the cost or book value of the securities. In your case, it would be the dollar value of the amount of shares.
