Do I provide exact name of person I paid for medical expense

You can group your expenses by generic names such as 'dental' or indeed summarize everything under one name. CRA doesn't care how you do it, so long as you have itemized receipts available for each claimed expense. CRA may ask you to submit receipts, so be sure to have them. Don't just claimed an arbitrary or 'guessed' amount as you will invite trouble.
