Will my wife get credit if she claims medical expenses even though her income was 0?

Medical expenses are a non-refundable tax credit that is used to reduce any net federal or net provincial tax your wife may have owing.  If your wife did not have any income, the medical expenses will not benefit her on her income tax return.
