A T4RIF I have received says "cancelled" with another identical T4RIF. However, the same amounts still stand on CRA website. Do I or don't I include it?
I have also received other T4RIFs but only the original that I have now received a "cancelled" T4RIF stands on the CRA website.

You should contact the Canada Revenue Agency to determine if they have an active T4RIF on file for you. If they do not, then you will not need to report it. The Canada Revenue Agency can be reached at 1 800 959 8281.
