I see you are using the TurboTax Do-It-Yourself version. I recommend signing out, clearing your cookies and cache, and/or trying a different browser. Also, try deleting and reentering the slip.
We suggest that you contact our product support team. They have access to tools we don't have here in the Community, like screenshare, and can investigate this issue for you.
