Some income on the T4A is considered to be self-employment income:

  • Box 20
  • Box 48
  • Box 28 and you selected yes to the question Does the income declared in Box 028 consist of fees collected for services you provided? in the online edition of TurboTax

If your T4A has one of these boxes, then you will have to fill out a T2125 - Business Statement. You only need to fill out the basics, and you can use your own name, address, etc… The only thing you might not know is what your industry code is, which you can look for here: https://www.statcan.gc.ca/eng/subjects/standard/naics/2017/v3/index
