Does my self employment address mean where I was located as a contractor for the work?

Your self-employment address will be the address you used when registering your business. For example, if you are a self employed consultant, and you registered your consultancy business in Ontario, your self-employment address will be Ontario irrespective of where you carried out contractual work(s). 

You can also enter your home address as your business address.
