• How to report pension  income from Polish government pension form PIT-40A/11A?  Any of it exempt?  Which line to report it on?  Thanks

You can report your foreign pension on the foreign slip. You can check the Tax Treaty between Canada and Poland to see if any is considered exempt or talk to someone knowledgeable on the subject.
