Your wife should have received a T4E slip for her E.I. Benefits received, regardless if they were regular benefits or for sickness.
To obtain a copy of the slip, you can log in to your "CRA My Account" (if signed up) and access the info there. Or contact Service Canada - 1 800 206-7218, or sign in to your "My Service Canada Account".
