Which boxes do I fill in under 'Other Employment Income Sources"?

I see the following:

Tips, gratuities or occasional earning not reported on a tax slip from the payer: 0.00

Other employment income
Specifiy: [ text box where I can enter the source]
Amount: 0.00

Do I entered the amount in the first box and the second box? Or just in one or the other?

It depends what the income you are trying to enter is. The first box is for Tips, gratuities or occasional earning not reported on a tax slip from the payer. You would enter the amount in one of the boxes only. If you use the Other employment income box, you will need to specify what kind of income you are declaring.
