The stocks are held in my Canadian and US accounts

Total value 5179.00

How do I claim this losses and carry forward with other capital gains losses

I have stocks that have gone to zero in value and I am unable to sell them

You can enter them in the capital gains section with proceeds of zero and cost of 5179.00.  You must have on file the proof that the stocks cannot be sold and that the company no longer exists or whatever documentation that will support your claim of zero value with not chance of recovery.  If the associated losses are not required in 2016 nor carried back they will automatically be carried forward.
